Oct. 15 (Bloomberg) -- "BP Plc's Texas City, Texas, refinery, where an explosion killed 15 workers in 2005, installed an improper metal-piping elbow that burst July 28 of last year and caused a hydrogen fire, U.S. safety regulators said today.
The two-hour fire that injured one worker and caused $30 million in property damage may have been prevented had contractors tested the alloy joint to ensure it was made of the correct material, said John Vorderbrueggen of the U.S. Chemical Safety and Hazard Investigation Board."
