BP incident prompts safety bulletin

The U.S. Chemical Safety and Hazard Investigation Board on Sunday morning issued a call for improved material identification systems in petrochemical facilities.

The report came as a result of a 15-month investigation into a fire and hydrocarbon release that occurred at BP’s Texas City refinery. That accident occurred four months after blasts tore through another unit at the same refinery, killing 15 people.
